PAYMENT POLICY

LABS2GO is an independent “fee-for-service” platform. We do not bill your insurance for our service's. You will be charged a one time “fee for service” directly for each ordered visit. We accept Credit Card and Debit Card, Cash payments ,Afterpay ,HSA AND FSA payments currently.
Your insurance information must be included on your physician requisition for your specified laboratory to process your specimens. Be sure your requisition is properly completed before you upload documents.

SCHEDULED APPOINTMENTS

We understand that delays can happen however we must try to keep the other patients and technicians on time.

If a patient is 20 minutes past their scheduled time we will have to reschedule the appointment.

I, the undersigned, consent to the collection of my health information for the purpose of blood tests and sample collection for diagnostic purposes by LABS2GO.

I authorize the release of my health information to relevant healthcare providers and laboratories involved in the testing process.

Privacy and Security:

I understand that my health information will be kept secure and confidential. LABS2GO implements security measures to protect my health information, including encryption and secure storage.

Retention and Disposal:

I acknowledge that my health information will be retaining 7 years and will be securely disposed of when no longer needed.

Patient Rights:

I understand that I have the right to access my health information and request amendments as necessary under the Health Insurance Portability and Accountability Act (HIPAA).
